Staff Vaccination/ COVID-19 Testing

Dear Roosevelt Union Free School District Employee,


Pursuant to the Determination of the Commissioner of the Department of Health all staff must be vaccinated against COVID-19 or else be tested for COVID-19 once per week.  In this regard, the Commissioner's Determination requires that all schools have the capacity to offer COVID-19 testing to teachers, students, and staff.  Therefore, the Roosevelt Union Free School District has contracted with a testing company to administer weekly, on-site COVID-19 screening testing, each Thursday at the Roosevelt High School from 3:00-5:00 p.m. 


The Commissioner’s Determination does not mandate vaccination against COVID-19.  However, teachers and staff can opt-out of mandatory weekly screening testing if they provide documentation of being fully vaccinated against COVID-19.


Each Employee must complete the District’s Vaccination Status Survey, provided below. Those who indicate that they are NOT Vaccinated, Prefer Not to Answer, or do not complete the survey, will be mandated to undergo COVID19 weekly screening testing, in accordance with the Commissioner's Determination.


To be eligible to opt-out of weekly COVID-19 screening testing, the Employee must show proof of full vaccination by providing a copy of their COVID-19 Vaccination Card or Excelsior Vaccination Pass to the building Principal, Central Office Administrator or school-based Nurse who is designated to collect this information.
